Estonia Data Source

The Estonian Health Insurance Fund administrative database

This database collects detailed and person-level billing data on all health care services provided to all insured population of Estonia from year 2000 onwards.

The database is owned by the Health Insurance Fund and data for research purposes has to be applied from the Fund by presenting the study plan and list of data necessary to answer the study questions. In case the proposed plan is approved, the requested data shall be extracted by the Health Insurance staff, and despite there is no fee, it can take considerable time to obtain the data requested.

Coverage

The data is collected since 2000 and it covers information on each medical contact (physician visit, hospital stay, etc) incl all diagnostic tests and treatment provided, as well as reimbursed prescription medicines.

Each out-patient or in-patient treatment episode includes the following data:

  • patient identification, date of birth and place of residence;
  • primary diagnoses and co-morbities (ICD-10), as assigned at the end of the treatment episode;
  • dates of all services provided;
  • list of all services provided (lab tests, diagnostic procedures, imaging, any treatment, surgical operations, bed-days etc
  • health care institution and responsible doctor;
  • amount and cost of each service provided.

In the EHIF database of reimbursed prescription medicines the type (INN name and ATC code) of medication can be identified as well as the amount purchased and paid by the EHIF and the patient (co-payment). It identifies also the date of prescription, date of purchase and the doctor, who issued the prescription.

 Linkage Linkage is technically possible, but is not done because of ownership and data protection reasons.
 Data quality

Presentation of data is complex and requires high expertise in payment system and rules applicable in Estonia as these change over time.

Quality of data is very good, as each entry is verified for billing purposes and checked for completeness and accuracy.

 Strengths and weaknesses

Strengths:

  • nationwide coverage and long time-series
  • detailed information on services provided and costs related
  • very rich information source on healthcare consumption and expenditure

Weaknesses 

  • data is not collected and available on:
  • patient health status and health determinants (weight, height, smoking habit);
  • results of lab analysis or any other diagnostic tests and investigations;
  • patients’ socio-economic characteristics.

 

The Health Statistics and Health Research Database.

This database collects annual reports from all health institutions in Estonia and presents statistical information, which can be online designed and abstracted according to the research questions in a comparable way over time and for international comparisons, i.e. per 100,000 population.

Coverage Collects and provides aggregate data on:
  • morbidity and disease incidence according to diagnosis;
  • use of health care services (out-patient visits, hospital care and emergency);
  • hospital discharges and hospital bed utilization;
  • provision of health care services (surgical operations, therapeutic procedures, diagnostic test and procedures)
  • the use of dental care;
  • health care providers (hospitals and primary care);
  • health care personnel;
  • health care expenditure;
  • equipment in health institutions;
  • investments and wages.

Data quality Since 1998, after the introduction of ICD-10 in Estonia, the data is well representative and comprehesive, but not comparable to previous years.
Strengths and weaknesses

Strengths – full coverage of al lrelevant health statistics fully online

Weaknesses – no linkage to health determinants and socioeconomic parameters

 

The Estonia Health Interview Survey

This is nationwide highly representative survey, which has been performed in 1996 and 2006 with the aims to describe the populations health status and provide quantitative information on health determinants and use of health services with regard to the sociodemographic and economic parameters.

Strengths and weaknesses Comprehensive dataset on health determinants of the Estonia population. Consistent questionnaire used over years, that has a lot of common modules with the European Health Survey used in other EU countries.

 

The Health Behavior among Estonian Adult Population database

The survey is carried out every 2 years since 1990 and it focuses on non-healthcare determinants of health.
